How do I customize the subscribe page for new members requiring they state their names and addys for moderator

We have a HOA website and we need to verify residency through tax rolls with Names and Addresses. I don't like delaying membership sign up because they did not read the intro page carefully and I have to email each one directly asking them to provide the info. Help!
